Risk Reduction Program
What you should know...
- The Camrose Primary Care Network (PCN) Risk Reduction Program offers specialized support for clients with chronic and/or complex health issues such as diabetes, high blood pressure, weight control and high cholesterol.
- The goal of the program is to help you remain as independent as you can be.
- There is no charge for participating in the program but a referral from your family physician is required.
How it works...
- In discussions with your family doctor, you may be referred to the Camrose PCN Risk Reduction Program where a team of healthcare providers will address concerns regarding your chronic health condition(s) including risk factors associated with pre-diabetes and/or heart disease.
- Your initial visit will take approximately 2 hours. Follow up visits may take up to 1 hour per visit. The Camrose PCN will call you to arrange for your initial and follow up appointment(s).
- After your initial assessment, you will work together with the team to develop a care plan unique to you. The team will also discuss the results of your assessment, recommendations and results with your family doctor.
Who you will see...
- The Camrose PCN Risk Reduction Program team consists of a registered nurse, pharmacist and dietitian.
- Additional healthcare and community providers may be consulted and referred to when needed.>
- Throughout your participation in the program your ongoing care will continue to be provided by your family doctor.
Where you will find us...
- The Camrose PCN Risk Reduction Program is offered at the Camrose PCN, main floor of the Edgeworth Centre, 4512 53 Street, Room 1110.
- Hours of operation are Monday to Wednesday from 8:30 am to 4:15 pm.
