The PCN Pharmacist |
Introducing the PCN Pharmacist!
The Camrose PCN is Proud to announce its newest addition, Phamie Gotaas! As the Camrose PCN pharmacist, Phamie will provide support to the existing physicians and pharmacists working in your area. In addition to providing the traditional services you currently rely on your pharmacist for, Phamie will be working actively in the community with patients, PCN physicians, and community pharmacies to improve the quality of pharmaceutical care you receive.
The PCN Pharmacist is here for YOU:
Education
As the PCN pharmacist, Phamie will be working in the community to bring you the knowledge you need in order to take your medications safely and effectively.
Access
By working actively in the community, the PCN pharmacist will give you the opportunity to give feedback on how your medication is affecting your health. This information will then be communicated back to your physician to assess the success of your medication plan.
Prevention
Many patients are unaware of the risks of taking a particular medication or become put-off by side effects and stop taking it all together, both of which are equally risky to your health. Patient education and accessibility to your pharmacist will prevent complications that may arise from taking medications incorrectly.
A Collaborative Approach:
The PCN pharmacist acts as a liaison between you, your physician, and your community pharmacists to promote communication and improve the quality of your pharmaceutical care.
The PCN Pharmacist will be working collaboratively with:
PCN Physicians
As well as following up with you on the status of your health, the PCN pharmacist will be working collaboratively with your physician offering medication assessments, drug information, and discharge planning recommendations.
AND
Community Pharmacies
As a liaison between community pharmacists, the PCN pharmacist will foster a collaborative approach to your pharmaceutical plan. This service will allow pharmacists to coordinate medications you may be receiving from more than one pharmacist, ensuring that you are on a safe and effective pharmaceutical plan.
Phamie can be contacted at the Camrose Primary Care Network Administration Office located in the Edgeworth Centre.
