About the PCN

Let's talk about Primary Care!

Primary care is your first point of contact with the health system. If you visit your family physician for a checkup, have your child immunized at a clinic, or are a resident of a long-term care facility, you are receiving primary care.

Taking Initiative with Primary Care

Alberta Government thinks your Primary Care is important! So much so, in 2003 they implemented the Primary Care Initiative. The main purpose of the initiative was to support the collaboration of physicians and healthcare providers in local areas into Primary Care Networks. So far, over 30 PCN's are operating throughout the province, and now, physicians and healthcare providers in Camrose have decided to get on board!

Camrose PCN:

A Cooperative Approach

Working within the PCN, your physician and other healthcare providers will be communicating and consulting with one another in order to come up with a health plan that is best suited to your needs. This means you will receive more interactive and more relevant health services as your health plan will be developed through the cooperation of a number of healthcare providers. And now, everyone will be kept up to date on the status of your health!

As a cooperative approach to your health needs, the formation of the Camrose PCN means that your physician and other healthcare providers (such as your dietician, and your pharmacist) are now working within the same network, towards the same goal - your health!

Health Services For You and Your Community!

As well as committing to a cooperative approach to your general primary care, the Camrose PCN has collaborated to develop additional services available for you and your family. This is a grassroots approach, meaning that services are developed by physicians and healthcare providers who work in your community and understand your needs. To better serve you and your community, the Camrose PCN has created additional services in the following five areas:

1. Health Promotion/Disease Prevention
2. Chronic Health Conditions (Diabetes, heart disease, high blood pressure, etc.)
3. Obstetrics (prenatal and postnatal care)
4. Health Supports for Seniors
5. Palliative Care (A special kind of health care for individuals and families who are living with a life - threatening illness, usually at an advanced stage)

Nothing But Results!

The additional services provided by the Camrose PCN will mean that answers to your health concerns are more readily available to you. This is because you can now receive a number of health services directly through the Camrose PCN; for example, to get information on a specific health topic or follow up after your doctor's appointment. You will also find getting referred is faster and easier as the PCN can now directly refer you to the healthcare provider, program, and/or specialist you need to see. And! You can still make an appointment to see your doctor just as you did before! In fact, with the PCN offering some of the same services, you may actually find that you have better access to your family physician!

1. You'd like information on a health topic, such as immunization of infants or home-care services
2. You think you might want to see a health care provider such as a dietician or a chiropractor, etc.
3. You want to follow-up after a doctor's appointment
4. You'd like a referral to a specialist
5. You need diagnostic services, such as laboratory work or x-rays
6. You'd like to sign up for a health related course or program such as prenatal classes, quitting smoking, etc.
