ABOUT THE PROGRAM

WHAT YOU SHOULD KNOW...

• The Camrose Primary Care Network (PCN) Risk Reduction Program offers specialized support for clients with chronic and/or complex health issues such as diabetes, high blood pressure, weight control and high cholesterol.
• The goal of the program is to help you remain as independent as you can be.
• There is no charge for participating in the program but a referral from your family physician is required.

HOW IT WORKS

• Your initial visit will take approximately 2 hours. After your initial assessment, you will work together with the team to develop a care plan unique to you. The Camrose PCN will call you to arrange your follow up appointment(s) and discuss the results of your assessment, recommendations and results with your family doctor.

WHO YOU WILL SEE

• A registered nurse, pharmacist and dietitian. Additional healthcare and community providers may be consulted and referred to when needed. Ongoing care will continue to be provided by your family doctor.
