    Colleen Lindholm | September 4, 2013

A letter sent to partners or stakeholders of the Camrose Primary Care Network:

As many of you know, the Camrose Primary Care Network (PCN) is preparing for its 2014-17 Business Plan submission. I am pleased to provide you with the following information and invitation to participate in one of our upcoming community stakeholder consultation sessions.

An essential part of the Camrose PCN’s vision is to: support family physicians in delivering coordinated, quality rural primary care with the support of other health professionals. At present, the Camrose PCN has thirty (30) family physicians registered practicing in Camrose, Daysland and Bashaw. The PCN also employs, seconds and/or contracts 25 health professionals and administrative staff.

Within our current Business Plan - the Camrose PCN identified five (5) priority initiatives: Prevention and Chronic Disease Management, Obstetrics, Geriatrics, Palliative Care and Medical Capacity. Each initiative has a number of programs and services attached that are either lead by the PCN or in partnership with community stakeholders. A listing of our current PCN menu of programs and services is attached.

In April of this year the Camrose PCN kicked off its 2014-17 Business Plan process and hosted 3 Town Hall meetings in Bashaw, Camrose and Daysland respectively. These sessions were meant to inform community members of the current PCN menu of programs and services as well as seek input regarding the patient’s experience, satisfaction and outcomes as it pertains to primary care in their community.

The Camrose PCN values all of our community partners. Therefore, the next step in our Business Plan process is to extend an invitation to your organization to participate in one of our upcoming community stakeholder consultation sessions. Sessions will take place the week of September 16th with a half day scheduled for each initiative from 9am – noon at the PCN Edgeworth Centre clinic location. The calendar is as follows:

    • September 16th    9am – noon     Prevention and Chronic Disease Management
    • September 17th    9am – noon     Obstetrics
    • September 18th    9am – noon     Geriatrics
    • September 19th    9am – noon     Palliative Care

The first hour will be used to review the PCN’s current Business Plan and menu of programs and services. The second hour will be focused on a SWOT (Strengths, Weaknesses, Opportunities, and Threats) exercise. Lastly we will put together a summary of what we heard during the morning and highlight any suggested recommendations regarding partnership opportunities for existing programming as well as new opportunities.

I appreciate that your organization may partner or have an interest in more than one of our PCN priority initiatives. Please identify and register only those individuals most appropriate for each session. Please register by emailing Pam Fankhanel at pam@camrosepcn.com.  For more information you may also contact Pam at 780.608.4927.

Enjoy your summer and I look forward to seeing you in September!

    Sincerely,
